SUBMIT A CLAIM
It is imperative to know the following with regards to submitting a claim:
- A claim must be reported to LVM and registered with the Insurance Company within 30 days of date of loss.
- In the event of a motor vehicle accident whereby you have no damage to your own vehicle, please advise and submit a formal claim to us, as the third party may submit a claim against you at a later date. This too must be submitted to us within 30 days of date of accident.
Windscreen Claims
In the event of a windscreen or side window claim, please fill in and submit the online claim form seen below or contact the claims department at LVM Insurance Brokers directly, who will then advise you accordingly and assist you with your claim.
Property Claims and Other Losses
For Buildings :
- Completed claim form
- Damage report, in the event of lighting, fire and / or water damage
- SAP case number, in the event of theft
- 1 x Quotation for the repair or replacement of loss incurred
For House Contents :
- Completed claim form
- Damage report, in the event of lighting or water damage
- SAP case number, in the event of burglary and / or theft
- 1x Quotation for the loss(s) incurred
For Other Losses :
- Completed claim form and damage report
- SAP case number in the event of loss or theft
- ITC (Blacklisting) number in the event of loss or stolen cell phone
- Repair / replacement quotation for a similar item(s) as the loss / damaged
Motor Accident Claims
Please note that you must make no admission or statement of liability or make any offer to any third party. In the event of a motor accident claim, the following items will be required in order to proceed:
- Completed motor accident claim form
- SAP case number for accident
- Driver’s licence of the driver whom was driving at the time of the incident
- 1 x Quotation for the repair of the vehicle
- In the event of a vehicle over 3500kg a copy of the driver’s PrDP (Public Driving Permit)
Motor Theft Claims
Please note that you must make no admission or statement of liability or make any offer to any third party. In the event of a motor theft claim, the following items will be required in:
- Completed motor theft claim form
- SAP case number for incident
- Driver’s licence of the driver whom was driving at the time of the incident.
- 1 x Quotation for the repair of the vehicle
- In the event of a vehicle over 3500kg a copy of the driver’s PrDP (Public Driving Permit)
- Original registration papers
- All keys of the vehicle (original and spare)
- Settlement letter from Bank (if applicable)
- Signed change of ownership. Download change of ownership form
Liability Claims
In the event of needing to request a liability claim, please contact our claims department at LVM Insurance Brokers directlywho will then advise you accordingly and assist you further with submitting your claim.
